- Healthy Mouth Plan: $400
- Periodontal Plan: $900
- * After a thorough examination by both the doctor and the hygienist, it will be determined whether you have a healthy mouth or signs of periodontal (gum) disease.
Healthy Mouth Plan
Patients with healthy gums and stable oral health are typically placed on a "Healthy Mouth Plan".
Periodontal plan
If there are signs of periodontal disease -- such as bone loss, deep gum pockets, bleeding gums, gum recession, or a history of gum infection -- the patient may be placed on a "Peridontal Plan".

- Periapical X-rays: Unlimited
- Periodic / Comprehensive Exam: 2 per year
- Complete Series X-rays (FMX): 1 per 3 years
- Bitewing X-rays: 1 set per year
- Prophylactic Cleaning: 2 per benefit year
- Periodontal Maintenance: 4 per benefit year

This is a dental membership plan, NOT a dental insurance plan.

Periodontal Plan: If there are signs of periodontal diseases, such as bone loss, deep gum pockets, bleeding gums, gum recession, or a history of gum infection, the patient may be placed on a "Periodontal Plan."
Program Limitations
- This is a dental membership plan, NOT a dental insurance plan.
- Cannot be used on cosmetic procedures.
- Cannot be used on products.
- Cannot be used for Teeth-In-A-Day cases.
- Cannot be used for referrals to specialists.
- Cannot be used for the costs of dental care, which are covered under auto or medical insurance.
- Cannot be used in conjunction with any other dental plan, cash discounts, or previous treatments.
- Cannot be used for services for injuries covered under workman's compensation.
